Children with Special Health Care Needs

Purpose

Children with Special Health Care Needs (CSHCN) is a program provides support and education to families. The CSHCN program promotes an integrated system of services for infants, children and youth up to age 18 years who have or are at risk for chronic physical, developmental, behavioral, or emotional conditions and require health and related services of a type or amount beyond what is generally needed. CSHCN allows us to facilitate in creating connections to state and county support for parents of children that may have unique health care needs. Referrals to this program is typically made by child’s provider or the progress center.
